29 CFR 1910.119(l)(1): The employer did not establish written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: a)Mill Creek Site, an electrical generator set, powered by a methane fueled internal combustion engine, was installed near the anhydrous ammonia engine room, and the changes to the process and equipment, were not addressed, on or about, February 27, 2015. b)Mahantango Site, O Cellar, piping and appurtenances to a tube and shell heat exchanger were changed from a motorized and remotely controlled valve to a manual valve and the changes to the process and equipment were not addressed, on or about, June 18, 2015. c)Mahantango Site, main ammonia engine room, a new compressor was added to the system and the changes to the process and equipment were not addressed, on or about, June 18, 2015. d)Mahantango Site, a new condenser was added to the system and the changes to the process and equipment were not addressed, on or about, June 18, 2015.
