29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies: (a) On or about 2/6/15 - At West Processing Buildings, the process hazard analysis did not address the engineering or administrative controls, procedures, or methods to ensure that the maximum limit of 500-lbs. of pyrotechnics and/or explosive compositions and a maximum weight of 10-lbs. of salute powder in loose form or in assembled units was not exceeded during processing and staging pyrotechnic compositions in multiple buildings such as A2 and A4 to A12.
