29 CFR 1910.217(c)(3)(vii)(a): When used in press operations requiring more than one operator, separate two hand controls were not provided for each operator, and were not designed to require concurrent application of all operators' controls to activate the slide. The removal of a hand from any control button did not cause the slide to stop. a.) Shiloh Industries, Inc. dba Wellington Stamping located at 350 Maple St. Wellington, OH 44090; The employer did not assure employees were protected from amputation hazards while operating the OBI Mechanical Power Presses, including but not limited to, the 01C OBI Danly Mechanical Power Press. On January 28, 2015, an employee reached into the press and sustained a 3 finger amputation when a second employee inadvertently cycled the press at the same time.
