29 CFR 1910.28(a)(6): A scaffold damaged or weakened was not immediately repaired: The employer does not ensure mid-rails are provided on a scissor lift that had a broken d-ring which is used to attach a chain style mid-rail. This violation was observed on or about November 25, 2014, in the structures department, exposing employees to the hazard of falls. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure adequate fall protection is provided for employees working on elevated platforms.
