29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a) Jobsite: Receiving, in front of the rail car - Employees receiving raw materials were exposed to trip and fall hazards from floor holes missing guards/covers. The floor holes measured approximately 8 feet long, 5 inches wide, 12 feet deep, and 8 feet long, 8 inches wide, and 12 feet deep, on or about 9/30/14.
