29 CFR 1910.147(c)(5)(ii)(B): Lockout and tag-out devices were not standardized within the facility in at least one of the following criteria: color; shape; or size. a. In Building 63, the lockout/tag-out (LO/TO) station locks were color coded red and the lock used on the cargo elevator switch was a master brand padlock. b. In Building 10, the lockout/tag-out (LO/TO) station locks were color coded red and in numerical sequence, the lock used for the Square D switch for the grinder was a small master lock with an "out of service" tag and the lock in use for the General Electric Main Switch was a long stem Ace brand padlock. c. In Building 95 electric shop, there were no locks in the lockout/tag-out (LO/TO) station and the lock in use on the Square D switch next to the office refrigerator was a master lock. d. In Building 63 and 95, the tag in use for the cargo elevator said "Do not start" with solid red color and other tags including the electrical shop tags were the standard white with red stripes that said "Do not operate". e. In Building 63, the "do not start" red tag on the cargo elevator switch was dated 2-9-11 and but did not identify the name of the person who applied the tag and a phone contact number.
