29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): On April 28, 2022, and times prior thereto, at this excavation site, employees were installing a sewer line lift station in an excavation with vertical walls. The excavation was 18-feet-deep, 64 feet 4 inches-long on one length, and it was 38 feet 5 inches-long on the second length of the "L" shape. The employer failed to provide cave-in protection by means of adequate sloping and benching system in accordance with OSHA requirements. Employees were exposed to struck-by/ engulfment hazards.
