29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads: a) On or about March 20, 2025, at the above addressed jobsite, a trench shield, used as a cave-in protection system in an excavation approximately six (6) feet eight (8) inches in depth, was installed in a manner that did not restrict the potential for hazardous movement in that backfilling was not performed along two sides of the shield, presenting gaps of approximately one (1) foot six (6) inches between the shielding system and trench walls.
