29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: (a) Basement; On or about June 19, 2024, and dates prior, employees were not given training on confined space entry, which includes understanding the hazards of confined spaces, the roles of entrants, attendants, and entry supervisors, procedures for atmospheric monitoring, rescue operations, and how to contact emergency services, prior to being allowed to enter the 6-ton mixer.
