29 CFR 1926.1427(b)(2): The trainer did not continuously monitor the operator-in-training on site while operating equipment: (a)Henry Carlson Construction, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the controlling employer did not ensure a trainer was provided and continuously monitored the operator-in-training. An operator-in-training was operating the Liebherr 550 HC40, 20-ton tower crane and was allowed to operate the tower crane without a trainer that would monitor and prevent a hazardous conditions related to improper operation. This condition exposed employees and subcontractor employees to falls, crushing, and struck-by hazards.
