29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: (a) CJ TMI Manufacturing America, LLC, waste water pit: The employer did not provide one or more attendants outside the waste water pit, a Permit Required Confined Space, when a Maintenance Mechanic entered to collect a water sample, on or about 11/24/21. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
