29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure: (a) On or about April 6, 2021, at a job site located West of Sentry Drive on West Sunset Drive, Waukesha, WI, spreader bar pins for the four trench boxes in the trench were not installed or fully inserted as required by tabulated data, to prevent lateral or other hazardous movement of the shield in the event of application of sudden lateral load on the trench box shield.
