Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about April 21, 2017, the employer did not ensure an employee working on the APF 5 was wearing metatarsal protection to protect the top of the employees foot from crushing injuries when handling aluminum ingots. b.
