29 CFR 1910.147(d)(4)(i): Lockout or tag out devices were not affixed to each energy isolating device by authorized employees: (a) At the sorter stacker: on or about June 24, 2013, and at times prior thereto, all employees who were clearing a jam and realigning the chain on the sorter stacker had not locked out the equipment or tagged out the equipment while they were exposed to the hazard of unexpected start up and movement of the chains.
