29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Core Room - An employee changed a core box on the CB22-SO2 core making machine. The employee only activated the emergency stop and did not de-energize the air and sulfur dioxide and did not apply lockout/tagout any of the three energy sources because the employer did not require employees to use lockout/tagout during this set-up activity, on or about January 11, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
