29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in the process complied with recognized and generally accepted good engineering practices: c) On or about February 24, 2010 - ammonia refrigeration machine room, the employer did not install an alarm within the machine room to warn employees of a failure of the mechanical ventilation system or ammonia monitoring; set at 1000 ppm to warn a failure. Good engineering practice as per ASHRAE 15-2004, Section 8.12(h) requires a ventilation system failure actuates an alarm or ammonia monitoring when the employer uses continuous ventilation in lieu of assigning the ammonia refrigeration machinery room a hazardous location designation of Class I, Division 2.
