29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about February 20, 2025, employees who were operating Toyota Electric Tow Motors while in the production area of the facility located at 1 High Tech Avenue, PAINESVILLE, OH 44077, were exposed to crushing and struck-by hazards in that the employer failed to certify each operator received refresher training including evaluation of the effectiveness of that training.
