29 CFR 1910.146(e)(1):Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: (a) 402 Tatamy Rd., Nazareth, PA - Dryer/Kiln: On or about January 23, 2025, employees engaged in welding, torching and grinding inside the dryer/kiln were exposed to entrapment and hazardous atmospheric conditions as a result of the employer's failure to to complete an entry permit.
