29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. At an establishment located at N4056 Vine Road, in Freedom, Wisconsin, (a) On or about April 24, 2024, in the Freedom 1 Area for Line 2, the Ultra Source packaging line had exposed cutting heads at the end of the conveying system with guarding which did not prevent contact from five unguarded cutter heads used to slice the length of the product packaging exposing employees to caught in between hazards. (b) On or about June 11, 2024, in the Freedom 2 Area, the Almac Moulder for Mixer 11 had a guard with opening large enough to access the augers below exposing employees to entanglement hazards. (c) On or about May 2, 2024, in the Ricotta Area, the Vertical Form Fill Seal Packaging Machine, Model: Elete, S/N: SB12689, had heat bars which were unguarded from below exposing employees to caught between hazards.
