29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about December 31, 2023, at the Mainline No. 2 Core Station, PA-134, employees performed servicing and maintenance activities such as, but not limited to, adjusting the chain on the top of the sprocket of the hoist table, without affixing lockout or tagout devices to each energy isolating device.
