29 CFR 1910.119(f)(1)(iv): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressed safety systems and their functions. (a) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not develop and implement a written operating procedure that provided clear instructions for safety systems and their functions. The "SAN WI 235 Loading HEPES Crystallizer T-30.03 with Methanol (effective date 9/01/2022)" operating procedure did not include clear instruction and function of the DR 3500 Radar Level transmitter and high level shut off. ABATEMENT DOCUMENTATION REQUIRED
