29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a- 40 Foundry Boulevard, Mount Holly NJ: On or about 09/03/22 a worker and the company worksite Foreman were exposed to fall hazards, in excess of 30 feet to the ground surface below, when an all terrain fork truck was used to support a single piece of OSB (reinforced with 2 x 4s) that they were working from. This single piece of OSB (reinforced with 2 x 4s) was was not attached to the forks of this fork truck. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
