29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On April 28, 2022, and times prior thereto, at this excavation site, employees were installing a sewer line lift station in an excavation with vertical walls. The excavation was 18 feet in depth, 64 feet 4 inches long on one length, and it was 38 feet 5 inches long on the second length of the "L" shape. The employer failed to provide cave-in protection. Employees were exposed to struck-by/ engulfment hazards.
