29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: (a) On or about April 6, 2021, at a job site located West of Sentry Drive on West Sunset Drive in Waukesha, WI, employees working in the fourth ( last trench shield box) were exposed to struck-by hazards from falling rocks, dirt, and concrete debris from the exposed 2 foot 6 inch high, by 20 foot long portion of the unprotected excavation face above the trench shield box, was not properly supported or barricaded to protect exposed employees working in the trench.
