29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source. On or about April 15, 2019, in the truss area, the employer did not ensure that all energy isolating devices needed to control the energy on the Alpine liner saw (Model #276c) was physically located and operated in such a manner as to isolate the saw from the energy source(s); this condition exposed the employee(s) to struck by hazards.
