29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about November 6, 2018, employees using circular saws and other electric power tools were exposed to electric shock hazards and the employer did not ensure that cord sets and power tool cords were provided strain relief to prevent damage to the equipment: a. The plug end of an extension cord used to power a DeWalt circular saw used on the south end of the building was not provided strain relief and was damaged. b. The plug end of a DeWalt circular saw used on the south end of the building was not provided strain relief and was damaged. c. The conductors entering a relocatable power strip powering battery chargers near the generator was not provided strain relief and the outer insulation was pulled away. d. The plug end of an extension cord used to power a DeWalt circular saw on the north end of the building was not provided strain relief and conductors were exposed.
