29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Throughout the facility - On or about June 7, 2018, and at times prior to, the employer did not properly train employees on the identification of energy sources, magnitude of energy sources, and the necessary steps for locking them out where employees were engaged in servicing, maintenance, unjamming, set-up, and cleaning equipment including but not limited to the Reivicart, Post, and the Double Feeder.
