Text For Citation: 01 Item/Group: 004C Hazard:
29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. On or about 01/25/2019 and at times prior to, employees performed servicing and maintenance on various machinery and equipment including but not limited to line-shaft conveyors and Peerless E-57 plate machines. The energy control procedures had deficiencies including, but not limited to the following: a. in the CV2 area, employees performing service and maintenance on equipment including but not limited to the conveyor lines were exposed to amputation hazards where specific procedures did not include the specific steps for the removal and transfer of lockout devices and the responsibility for them. b. at the Peerless E-57 plate machines, employees performing service and maintenance were exposed to amputation hazards where the specific procedures did not include specific steps for the removal or transfer of the lockout or tag out devices and the responsibility for them. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. The Aspen Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1050453, citation number 1, item number 1a and was affirmed as a final order on 07/07/2015, with respect to a workplace located at 3220 Avondale Mill Rd in Macon, GA 31206.
