29 CFR 1910.147(d): At this establishment, the established procedures for the application of energy control (the lockout and tagout procedures) did not cover the following six elements in the following sequence: (1) Preparation for shutdown (2) Machine or equipment shutdown (3) Machine or equipment isolation (4) Lockout or tagout device application (5) Stored energy, and (6) Verification of isolation. (a) Established energy control procedure for the "OscamSlitter" at Line #10 in the Production Area, did not adequately cover the following steps: (step 1) "Preparation for shutdown" was not adequately covered in that the magnitude(s) of hazardous energy were not listed. This procedure made reference to Electrical energy but did not list its magnitude. This procedure also made reference to air pressure or pneumatic energy but did not list its magnitude. (step 3) "Machine or equipment isolation" was not adequately covered. The procedure specified: "The E-1 Disconnect is located in the south side of the machine on the control box. Turn the disconnect switch clockwise to the off position and lock out at E-1." However, the disconnect switch actually must be turned counter-clockwise to be in the off position. (step 6) "Verification of isolation" was not adequately covered. The procedure stated: "Verify total de-energization of all sources", but no procedural step(s) were included for testing the equipment to determine and verify the effectiveness of the lockout devices and energy control procedures.
