29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) Employees were exposed to amputation hazards from the 500 machine manufactured by Vicon Machinery. Employees were exposed to ingoing nippoints from the belts and pulleys which were not adequately guarded within seven feet of the floor. Conveyor sections of the 500 machine did not have the pulleys adequately guarded. On or about February 3, 2017 an employee received an amputation injury of their finger when their hand got caught in the rotating shaft, and horizontal belts and pulleys while operating the 500 machine. (b) Employees were exposed to amputation hazards from the Old Collar Line manufactured by Vicon Machinery. Employees were exposed to ingoing nippoints from the belts and pulleys which were not adequately guarded within seven feet or less from the floor. Conveyor sections of the Old Collar Line did not have the pulleys adequately guarded.
