29 CFR 1910.27(c)(4): The employer did not install a fixed ladder so that the nearest permanent object in back of the ladder shall be not less than 7 inches, except when unavoidable obstructions are encountered, the ladder shall be installed with clearances as shown in figure D-3. From figure D-3, the employer did not install the fixed ladder so that there was at least a 4 1/2 inch minimum clearance between the obstruction and the centerline of the rung just beneath the obstruction: Q1, Q Line: On, or about, July 1, 2016, an obstruction was located at the fourth rung on the lower access ladder.
