29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: (a) Paint Booth: The employer did not conduct a hazard assessment of the respiratory hazard(s) in the workplace before requiring an employee to use a 3M, half mask negative pressure air purifying respirators with Organic Vapor Cartridges, during spray painting operations using PPG Industrial Alkyd Enamel, which contains Xylene, as determined on or about 07/18/2014. (b) Sand Blaster Area: The employer did not conduct a hazard assessment of the respiratory hazard(s) in the workplace before requiring an employee to use a supplied airline hood, during sand blasting operations using Black Beauty, which contains Silica, as determined on or about 07/18/2014.
