29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employee's name and dates of training: (a) On or about June 24, 2013, and at times prior thereto, the employer could not provide certification that employees that were re-aligning the lumber carrying chain had been trained in lock out tag out requirements.
