29 CFR 1910.119(e)(6): The employer did not ensure that the process hazard analysis was updated and revalidated by a team meeting the requirements in paragraph (e)(4) of this section at least every five (5) years after the completion of the initial process hazard analysis: (a)Hill Air Force Base, at 7290 Weiner Street, Hill Air Force Base, UT: On and preceding 3/16/11, Hill Air Force Base did not ensure that the process hazard analysis was updated and revalidated by a team meeting the requirements in paragraph (e)(4) of this section at least every five (5) years after the completion of the initial process hazard analysis. The wastewater treatment plant, Building 575, used sulfur dioxide to reduce chromium (VI) to chromium (III). The employer maintained approximately four thousand pounds of liquified sulfur dioxide in the system. The employer completed the initial process hazard analysis in 1998. The employer did not ensure that the process hazard analysis was updated and revalidated at least every five years since the initial process hazard analysis.
