29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in the process complied with recognized and generally accepted good engineering practices: b) On or about February 24, 2010 - ammonia refrigeration machine room, the employer did not set ammonia sensors within area of the machine room where ammonia could accumulate at 50 ppm to warn employees of leaks from ammonia refrigeration equipment. Good engineering practice as ASHRAE 15-2004 Section 8.11.2.1 requires an ammonia detector that actuates an alarm and mechanical ventilation with a set point to be no greater than the refrigerant TLV-TWA (ammonia TWA= 50 PPM).
