29 CFR 1926.451(f)(7):Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) Jobsite located at or near 5800 City Ave, Philadelphia PA - On or about June 24, 2025, the employer did not ensure that employees working from an outrigger platform attached to a Fraco ACT 8 Mast Climbing Work Platform were protected from fall hazards of approximately 60 feet to a lower level by having a competent person ensure that a guardrail was installed on each end of the platform. b) Jobsite located at or near 5800 City Ave, Philadelphia PA - On or about June 24, 2025, the employer did not ensure that employees working from an outrigger platform attached to the fabricated frame scaffold were protected from fall hazards of approximately 60 feet to a lower level by having a competent person ensure that a guardrail was installed on each end of the platform. c) Jobsite located at or near 5800 City Ave, Philadelphia PA - On or about June 24, 2025, the employer did not ensure that employees working from fabricated frame scaffold were protected from fall hazards of approximately 60 feet to a lower level by having a competent person ensure that a guardrails were installed on all open sides.
