29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards. Location: Worksite On or about March 19, 2025, employees were exposed to fall hazards of over 40 feet while working from the fabricated frame scaffolding in order to install the new copper downspout, where the employer did not ensure that employees had been trained by a qualified person in order to be able to recognize the hazards associated with the fabricated frame scaffolding being used and to understand the procedures to control or minimize those hazards.
