29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: a) On or about March 20, 2025, at the above addressed jobsite, the Pro-Tec PRO6-424D trench shield Serial Number M0401458 installed in an excavation approximately six (6) feet eight (8) inches deep was not constructed in accordance with the manufacturer's tabulated data where only one spreader pipe was used at the south end, thereby exposing employees to struck-by and crushing hazards.
