29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (I)(4)(ii) to ensure that the operator has knowledge and skills needed to operate the powered industrial truck safely: On or about February 20, 2025, employees operating Toyota Electric Forklifts within the production area of a facility located at 1 High Tech Avenue, PAINESVILLE, OH 44077, were exposed to crushing and struck-by hazards in that the employer did not conduct refresher training and/or an evaluation of the effectiveness of training to ensure the operator had the knowledge and skills needed to operate the powered industrial truck safely.
