29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about April 9, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; the pop-up guard on Notcher #1 created a caught-between hazard when it returned to the home position after the machine cycled. The upward movement of the guard created an approximate 1/2-inch gap between the guard and the workpiece clamp when employees reached in to remove wood boards.
