29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. At an establishment located at W3955 Mill Road in Bryant, Wisconsin, employees were exposed to caught-in hazards created by the rotating projecting shafts in the following instances; a) On or about January 8, 2025, on the green chain, both ends of the green chain conveyor has projecting shaft ends that are projecting more than one half the diameter and were not smooth; b) On or about January 8, 2025, on the edger conveyor there is a shaft end projecting more than one half the diameter and was not smooth.
