29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) 1000 CenterPoint Blvd., New Castle, DE 19720 (OCC-1 Compressor Thermosyphon Oil Cooler/Shell and Tube Heat Exchanger) On or about August 15, 2024, the employer did not document that equipment complied with their chosen Recognized and Generally Accepted Good Engineering Practices (RAGAGEP) ANSI/ASHRAE STANDARD 15 -1989 Safety Code for Mechanical Refrigeration section 9.1.6 when the employer failed to ensure liquid ammonia and/or oil from relief devices were directed to a safe location. Failure to install relief devices in the proper locations could result in an employee's exposure to vapor or liquid ammonia resulting in serious injury. b) 1000 CenterPoint Blvd., New Castle, DE 19720 (OCC-2 Compressor Thermosyphon Oil Cooler/Shell and Tube Heat Exchanger) On or about August 15, 2024, the employer did not document that equipment complied with their chosen Recognized and Generally Accepted Good Engineering Practices (RAGAGEP) ANSI/ASHRAE STANDARD 15 -1989 Safety Code for Mechanical Refrigeration section 9.1.6 when the employer failed to ensure liquid ammonia and/or oil from relief devices were directed to a safe location. Failure to install relief devices in the proper locations could result in an employee's exposure to vapor or liquid ammonia resulting in serious injury. d) 1000 CenterPoint Blvd., New Castle, DE 19720 (Rooftop-Relief Header/diffuser): On or about August 15, 2024, The employer did not document that the equipment complies with section Appendix A section Diffuser B .7 (pg 20) of ANSI/ASHRAE STANDARD 15 -1989 Safety Code for Mechanical Refrigeration when the employer did not ensure the rooftop diffuser design complied with section 9.4.8 which states the discharge termination shall be fashioned in such a manner to prevent direct spray discharge refrigerated on personnel in the vicinity. This hazardous condition exposed employees accessing/egressing from the roof using the fixed ladder system to severe injury due to the potential of fire and toxic chemical overexposure hazards during an anhydrous ammonia relief discharge event.
