29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources. a) 731 Avenue H Columbia, PA 17512 and 615 Locust St. Columbia, PA 17512 - Facility Wide - On or about August 8, 2024, employees were conducting service and maintenance of machinery used for glass etching and painting including, but not limited to, laser engravers, CrystalBlast 3624 sandcarving equipment, and Pad Printer and the employer did not provide breaker adapters for the locking out of the circuit breakers within the panel boxes. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
