29 CFR 1910.184(h)(5)(v):Natural or synthetic fiber rope sling(s) with discoloration or rotting were not immediately removed from service: Employees engaged in material handling operations located at 14961 Shepard Street, Omaha, Nebraska 68138, were exposed to struck-by hazard in that an employee used a discolored nylon sling for rigging a grapple attachment in the welding bay. The discolored nylon sling failed when suspended and struck and employee in the leg. This was most recently documented on or about May 24th, 2024.
