29 CFR 1926.651(k)(1): Inspections of excavations, the adjacent areas, and protective systems were not made by a competent person, prior to the start of work and as needed throughout the shift, for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: On or about April 29, 2024 and at times prior thereto, on the corner of E 134th St S & S Lewis Ave, an employee was performing pipeline installation work activities in the middle of a trench that was not inspected by a competent person.
