29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury, the defective or damaged item was not removed from service, and an employee was using it before repairs and tests necessary to render the equipment safe were made: On or about April 3, 2024, fabrication employees in the Value Added Department of Greater Omaha Packing Company, Inc. located at 3001 L Street, Omaha, Nebraska were exposed to electrical burn and hazards in that the employees operated the Signode HBX 4300 strapping machine with a damaged cord and exposed, live wiring.
