29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a) Ferraro Fine Foods Corp., 1301 Broad Street, Utica, NY - Throughout the Warehouse. On or about March 13, 2024 a power industrial truck operator was tasked to perform material handling operations without receiving refresher training, including an evaluation of the effectiveness of the training after being involved in an incident which resulted in serious injuries.
