29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about December 28, 2023, and at times prior, authorized employees did not receive training on the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation associated with the machinery including, but not limited to, the Maintenance Dock's LD 6000 XD/DUMPMASTER Cardboard Dumper and Galbreath Compactor 1 (CMP1). As a result, employees were exposed to hazards such as, but not limited to, contact with electrical, struck-by, and caught-in/between hazards associated with the pinch points, in-going nip point, moving components, and live electrical components associated with the equipment during maintenance and servicing tasks. These tasks included, but were not limited to, making repairs, troubleshooting, and maintaining the various components and parts of the dumper. b. On or about November 26, 2023, and at times prior, authorized employees did not receive training on the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation associated with the machinery including, but not limited to, the Universal Sorter Machine. As a result, employees were exposed to hazards such as, but not limited to, contact with electrical, struck-by, and caught-in/between hazards associated with the pinch points, in-going nip point, moving components, and live electrical components associated with the equipment during maintenance and servicing tasks. These tasks included, but were not limited to, making repairs, troubleshooting, and maintaining the various components and parts of the Universal Sorter Machine.
