29 CFR 1910.184(e)(2)(ii): Makeshift link(s) or fastener(s) formed from bolts, or other such attachment(s) were used to connect links or fastenings on alloy steel chain sling(s): a) On or about October 17, 2023, a makeshift alloy steel chain link comprised of bolts and shackles/clevis was utilized on the 1Ton Gorbel overhead Gantry Crane located in the assembly area. The rated capacity of the sling was compromised and exposed employees to a hazard from suspended loads. b) On or about October 17, 2023, a makeshift clevis comprised of bolts to hold the load was used on the 1/2Ton Gorbel overhead Gantry Crane located in the assembly area. The rated capacity of the shackle/clevis attachment was compromised and exposed employees to a hazard from suspended loads.
