29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about August 15, 2023, employees operating machines in the assembly area were exposed to crushing/struck by hazards (from the press) at the point of operation of the DENISON Multipress; the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle. b) On or about August 15, 2023, employees operating machines in the assembly area were exposed to crushing/struck by hazards (from the press while manually holding the material directly next to press) at the point of operation of the spring and ball press; the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle. c) On or about August 15, 2023, employees operating machines in the stock cutting area were exposed to caught in/caught by hazards (from the dies), rotating parts (the 4 dies), and flying chips (from the material) at the point of operation of the ZAGAR INC. multi-spindles; the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle. d) On or about August 15, 2023, employees operating machines in the manufacturing area were exposed to the rotating parts (drill bit and chuck), and flying chips (from the material being drilled) at the point of operation of the drill press(11D974); the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle.
